You can get severe muscle cramps, or heatstroke if you don't have sufficient electrolytes. When you sweat, you lose electrolytes, primarily sodium, but also magnesium and calcium. These tablets are formulated to replenish the minerals in the correct proportion. Sports drink don't do nearly as good of a job and usually have far too much sugar. If they had enough electrolytes, they'd taste awful. If you remember the original Stokely VanKamp Gatorade, which came in glass bottles, it didn't taste very good, but it was effective. The new owners changed it to make it more palatable, but that made it much less effective.
