Can I use this tool on an oak tree to pressurize a treatment with a pressure tank?

It should work if you’re using the same level of pressure as your existing water line would supply; too much pressure and it would probably leak at its seals.

How far away from a tree should I insert this? I don't want to damage the roots by inserting too close to trunk

The roots on most trees spread out as far as the branches. Think of the tree upside down and you will have an idea of what the roots look like. I go half way between the trunk and the branch tip. The only thing that might be a problem is if there is poor drainage. Standing water will kill the tree so don't get it too wet.

How many holes are there? This site gives contradictory information. 3 holes (two on side , one at bottom) and 4 holes. (placement?) Which is is?
