It's painful, my rotator cuff is tight with Inflammation, and I can not raise it up much higher than a handshake, so I needed a shoulder ice pack. I have regular square, flat ice packs that are hard to wrap on my shoulder, so I needed this. It helps being able to walk around with an ice pack and not worry about it sliding off. I have not put it on alone though, so I'm not sure if I can get it on alone, and it stays cold on my arm with the velvet side down for almost an hour. I could turn it over onto the frosty side with a towel on my arm, but it lasts long enough on the one side. I do put it back in the bag when I put it back into the freezer, and I recommend it.*I wasn't in a stabilizer sling when I wrote the first review, and I had to have a revision repair on my shoulder with a different surgeon 2weeks ago due to the first surgeons incompetency. But, I can get in and out of this by myself, and I love it. You just have to put it on gently, because your shoulder will be sore, but the ice packs at physical therapy are a bit stiff at first also, so it's normal. I wanted to show you how it looks with a stabilizer sling on. My surgeon gave me a similar cheaper free square ice pack after surgery, but I only use this one without a second thought. It wraps well on the areas that need the ice the most, front shoulder, back of the shoulder, and down the outside of the arm. I have problems with my shoulder and it is hard for me to use this wrap without alot of help. Once it is on, it works great and does a good job of applying cold or heat to my shoulders. I have a fairly large chest, male, and the stretch belt that goes around from the back to the front under the opposite arm is very hard to get completely around and fastened. It takes both me and my wife several times to get it to stay. The other wrap hat secures the other end toyour upper arm is also impossible to to alone. It is easier to wrestle 2 cats into a bucket of water, lol.With that said, this product does the best job for applying cold or heat to my shoulders. No more fighting to keep a bag on my shoulder or having to sit still, afraid to move and loose the bag. For me, it is worth the effort to put on. My wife may have a different opinion.
