It is a plastic cover to go under the cloth cover. It only covers the big part of the pillow, but it doesn't need to cover the rest because you're unlikely to get milk on that part. I find it essential to prevent milk from seeping into the pillow and making it stank. I have a pretty fast flow and good supply so either I get milk on the pillow or my baby spits up on it. It doesn't make the pillow crinkly or annoying, and it makes it easier to get the cloth covers on and off. The one disadvantage is that since the milk doesn't seep into the pillow it stays wet, but you can just take off the cloth cover and hang it up to dry (or wash it).
