This works really well to hold a cold pack exactly where my pain is. The pack that comes with it is a small, thin, gel pack. I use it to start- it lasts about 25-30 minutes before it's not cold enough to help. Then I switch to a thicker gel pack. I use gel so it's soft and not sharp on pain. Gel doesn't stay cold as long as some of the other types of cold packs though. If you have severe pain, it's way soft and flexible so you may just want to have multiple so you can keep swapping them out.I put a loose scarf on my head as I'm bald and use the mesh side so the ice is directly on my head. The fabric side is too thick to allow the cold to penetrate effectively. I use it often. ### ⭐⭐⭐⭐ 4.0 out of 5 stars Works very well, provides dependable relief *by W***N on Reviewed in the United States on February 6, 2016* To me this is really TWO products; the Velcro elastic strap and the freezable gel pack with the fabric wrapper. I have found much better freezable gel packs and the fabric wrapper is too small for most other gel packs, so I don't use anything but the Velcro elastic strap. That said, the Velcro strap alone is worth the modest price. I suffer from frequent headaches and have for many years. I used frozen gel packs wrapped in a handkerchief tied to my head to get the kind of hands-free relief I needed to continue to work (alone at home, it looks ridiculous). With the Velcro strap and any microfiber cloth I can fold the gel pack into the cloth, choose the side with either one or two layers of cloth (which lets me have the coldest or the not-quite coldest contact with my forehead) and have it all held smoothly and securely in place. This system works so well that, when my daughter discovered this treatment for headaches, I gave her my Velcro strap and bought another one.
