Does this come with the entire caddy or just the metal 3.5"->2.5" insert?

The entire caddy, but it will only work for a 2.5 drive.

What are the black rubber pieces that come with it for?

The black rubber piece can be place directly under where the 2.5" drive is mounted on the HDD carrer tray. I believe the purpose is for providing cushioning and stability for some of the 2.5" drives. Is this the whole tray or just the plate to mount the SSD to?

It's the whole tray, in that you don't need anything else to mount a typical 2.5 SSD. But if your SSD required an adapter (e.g. some of the ultra thin ones,) then you will need that adapter as well. But if your SSD didn't come with an adapter, you won't need one here. This tray slides into the 3.5 slot.
