I use it for sleeping on my back. I had tried the Silx double-filled version but it was too thick and gave me neck pain every day. One reviewer commented that it was like sleeping on a towel -- yes in that this pillow is flat like a folded bath towel, no in that it's softer than sleeping on a bath towel (believe me, I was using a bath towel for several nights).I like this pillow because it's the only one flat enough (that I found, but not too flat) that gives me the correct neck-spine alignment. It's not the same as not having a pillow -- the head and neck would be at a different angle (I tried sleeping without a pillow as well). I am using it after a spine injury. Normal people may use it for sleeping on their tummies. If you happen to like really flat pillows I'd recommend you give it a try.Cons: (1) there is a smell when it's new but it dissipates after several days (2) it requires re-fluffing every evening and the filling is prone to shift.I just purchased the Boudoir size so as to use for traveling.Update 1/12/2016: I lowered the pillow from 4 stars to 2 stars. The pillow scrunches up in the middle of the night and causes my head and neck to scrunch with it, causing horrible neck pain by morning. I've since switched to using a folded fleece blanket .. the polyester fleece has some bounce which makes it much more comfortable than using a folded towel. We like the idea that silk is naturally resistant to dust mites. However, after trying it out for a few minutes we had to return it. My wife wants a flat firm pillow. This pillow is flat, but it is very soft and squishy. So when lying on your back it is almost like not having a pillow at all. We were disappointed because we really wanted to like this one. My wife sleeps on her back and side.
