I don't use diaper cream, but make sure you use a cloth diaper friendly cream and give it a whirl. The liners are thick enough to contain poo and thin enough to let the wetness go thru to the padding. I imagine a cloth diaper friendly cream would do its job on baby's bottom and the liner would do its job as well. **Q: Can you use liners with underwear** A: You could.
