How do you attach this to a camera/lens? I just received this and there was no plate in the box.

The FlexShooter Pro has an Arca-Swiss style receiver clamp, making it compatible with a broad range of quick release plates, brackets, lens foot plates, and other compatible accessories.

Does it work with Monopod?

This won’t really work with a monopod (not the way it is designed to work). I guess you could physically attach it to a monopod, and keep your camera secured, but the purpose of the dual ball-head is to level out the camera and keep it focused where you want. The movement of the monopod would eliminate the primary function of the FlexShooter.
