Truth is, I can't guarantee a non-falling head. But I can say with assurance that head instability will be greatly reduced if you do the following:1. Position your elbows in the vaVesta sleeves at a 90-degree angle or less.2. If necessary for your size (usually 5' 4" or shorter), shift vaVesta on your neck to achieve the 90-degree or less elbow position. Do this by aligning the bottom "sleeve" on one side with the top "sleeve" on the other side.3. As your head leans back into vaVesta's neck support, the slight downward pull of your arms will stabilize your head.4. To increase head stability, cross one side of vaVesta over the other (see photo). Crossing vaVesta in this way is the ideal sleeping position.5. Second, when a person's head is resting against this Insert and their arms are resting in vaVesta's sleeves, a counterbalance is created that effectively stabilizes the neck and head. It doesn't look like there is any actual support for the head.** A: There are a number of different ways you can use the VaVesta. I've found the best way for supporting the neck is by crossing the front parts over (as shown in several of their pictures), which provides a good level of support. It's not perfect, but it's a massive reduction to the amount of strain otherwise applied to your neck. I'm flying to Brazil in two days and really looking forward to using it again - one of the most helpful things I've found is how I can use it to support my arms (I am very broad shouldered) in those horrendously tiny little excuses for seats the airlines cram in. **Q: Is there supposed to be something like a zipper or flap that goes over the pillow insert in the back?** A: No, it's an opening, narrower than the insert is long but wide enough for easy insertion and removal. I've never even come close to having the insert fall out, regardless of direction.
