It’s very flexible and is foldable but would not stay folded unless stuck to something.

Can this be used on skin like silicone scar tape?

Not a professional, but I would say no. Item wasn't packaged in sterile condition.

Will this work as a cell phone grip? Is it adhesive on either side?

It has adhesive but it’s not designed for that use and it will more than likely peel off as soon as dirt works it’s way into it, I wouldn’t recommend this particular use of the product.
