**Q: Is the version sold by you (rev 12/10/07) accepted by the fbi for generating identity history summary checks?**
A: I did use these cards for submission to FBI for history summary. No problems.

**Q: Is this the new version or old one?  I need the current one for a permit application.**
A: The version we sell is Rev 12/10/07 - which is accepted for your requirements.  We are a professional fingerprinting company in Illinois and we use this form for all of our clients.  Between livescan and ink fingerprinting we print more than 1200 people per month.
