I've used them hot and they also work pretty well, but they can get EXTREMELY hot, and over time, the gel breaks down and they become a lot less effective when used cold. I have one for hot and save the rest for the freezer. For the price, you really can't beat them. Buy at least two. The plastic is tough and well seatle. I can even lie on it and it won't rupture. The pack stays cold for more than two hours. I'm glad they're finally offering this pack with a cover. The gel pack is VERY cold when it comes out of the freezer and needs to be wrapped in something before use. The cover's material is far from harsh but it could be softer and thicker. I highly recommend this product.
